Information for New Patients

For Patients

To initiate treatment at The Psychological Center, prospective patients must complete an online information packet. The online information packet is to be completed by a legal guardian for all prospective patients 17 years and younger (minors).

Once the packet is complete, our Intake Coordinator will call you to schedule an initial Screen Appointment. Screen Appointments are conducted in-person only. Individuals and/or legal guardians (parents/caregivers) of minors interested in receiving psychological or neuropsychological testing must attend their screen appointment in-person.

The Psychological Center provides predominantly in-person services. Virtual services are available upon request on a very limited basis. If you’re interested in virtual services you will have the opportunity to discuss this with the Intake Coordinator after completing the online packet. Please be sure to select the appropriate paperwork option: Child Services or Adult Services. All prospective patients must present an ID to CCNY Security at the building entrance when they come to our Clinic.

For Providers

Providers should instruct potential patients complete the online information packet. Providers cannot schedule this appointment for the potential patient. The Psychological Center will not contact the patient to schedule an appointment for screening until the prospective patient completes the online information packet.
